The respirator idea with the heartbeat is overdoing it. DJ Z is right about this being more of an experimental song than a ready-for-radio song. I don't want to give this a low rating because I like Lil' Kim, but this was an atrocious song; in any case, welcome back from your mini hiatus LK.
